August 1, 2018NYS Department of Health Awards St. Catherine’s 5-year, $2.5 Million Health Home Supportive Housing Grant
The New York State Department of Health has awarded St. Catherine’s Center for Children a five-year, $2,500,000 Health Home Supportive Housing grant. The grant will be used to extend St. Catherine’s Project HOST, a program launched in 2014.
“St. Catherine’s is incredibly pleased to receive this grant,” said agency executive director Frank Pindiak. “The application process was extremely competitive, with organizations from around the state seeking support for their supportive housing programs. Our selection as a Capital Region grant recipient not only recognizes our strong history of service to the homeless community, but it also reflects faith in our ability to continue providing services at the very highest level.”
The program grant is designed to provide supportive housing and other vital services for homeless or unstably housed Medicaid members who are enrolled in New York State’s Health Home Program. Supportive housing is a simple and common sense solution designed to serve vulnerable people threatened with homelessness. Supportive housing combines affordable permanent housing with services that help people facing the most complex challenges live with stability, autonomy and dignity. Services are individualized, accessible, flexible and include referrals and linkages to community resources.
St. Catherine’s Project HOST (Healthy Outcomes and Supported Transitions) began as a pilot program four years ago, when St. Catherine’s received a $400,000 Department of Health grant to launch the initiative. Since then, Project HOST has provided critical supportive housing services to more than 200 adults in the Capital Region.
