CDPHP Partners with Albany Medical Center to Offer Patients, Families Enhanced Hospital Experience

CDPHP, in partnership with Albany Medical Center, is thrilled to announce the expansion of CDPHP Hospital to Home, a concierge-level service designed to keep patients and their loved ones supported and well-informed during a hospital admission.

After operating virtually for the better part of the last year due to the current COVID-19 pandemic, CDPHP Hospital to Home staff are back at the bedside to coordinate care, answer questions, provide updates to loved ones, assist with medication management, and collaborate with Albany Med staff on discharge planning for CDPHP inpatients.

“Being admitted to the hospital can be confusing and overwhelming, and never was that more true than over the last year, in the midst of a once-in-a-lifetime global pandemic,” said John D. Bennett, MD, president and CEO of CDPHP. “I am thrilled that our Hospital to Home team is returning to the bedside at Albany Medical Center to support inpatients and their families. CDPHP continues to be more than an insurance company, and this is yet another example of that commitment.”

Hospital to Home is an important step in the CDPHP vision of creating an integrated delivery system, which, at its core, creates a seamless care experience for patients between all of their care providers and CDPHP as their insurer.

The ribbon was also cut on our sixth CDPHP Customer Connect location within Albany Med, where anyone can get their benefit questions answered and enroll in CDPHP health plans.
