Capital District Physicians Health Plan (CDPHP) is conducting a second Open Enrollment period in June 2021 for an effective date of July 1st. Current and new subscribers have the opportunity to change from one CDPHP plan to another, from a different carrier to CDPHP, or for new subscribers to enroll for the first time.
